Gifted Hands Community Health (GHCH) is a community-based organization dedicated to improving health, independence, opportunity, and quality of life for individuals and families throughout New Jersey.
Our work extends across community health and wellness, supportive and developmental disability services, therapy and clinical supports, workforce development and career pathways, education, and community outreach. We connect individuals and families with services, resources, training, and opportunities that support healthier lives, greater independence, and long-term success.
Through person-centered services, health education, preventive health initiatives, skill development, and strong community partnerships, GHCH works to address the unique needs of the people and communities we serve.

Health, Wellness & Education
GHCH promotes healthier communities through health education, wellness initiatives, prevention, resource navigation, and community outreach. We help individuals and families gain the knowledge and resources they need to make informed decisions and improve their well-being.
Independence & Supportive Services
Our person-centered services help individuals build skills, increase independence, participate in their communities, and achieve meaningful personal goals. Services include developmental disability supports, therapy and clinical services, and individualized assistance based on each person’s needs.
Workforce & Community Development
GHCH creates pathways to opportunity through workforce development, healthcare career education, community partnerships, and connections to resources. We work with families, professionals, employers, and community organizations to strengthen individuals and build healthier, more resilient communities.
Physical and Occupational Therapy
GHCH provides individualized Physical and Occupational Therapy services designed to improve mobility, functional abilities, daily living skills, independence, and participation in home and community life. Services are guided by each individual’s assessed needs, goals, and plan of care
Individual Supports
GHCH helps individuals develop and maintain skills for greater independence at home and in the community. Supports are individualized and may focus on daily living skills, community participation, personal goals, self-direction, and greater independence.
Community Health & Wellness
GHCH promotes healthier communities through health education, wellness programs, preventive health initiatives, screenings, health literacy, and connections to healthcare and community resources. We work with individuals, families, and community partners to improve access, awareness, prevention, and overall well-being.
